Dental Coverage

Dental benefits for NCSMIG are provided via contract through a national contract with Guardian Insurance. While you are free to use any provider of your choice, your plan will pay the highest benefit if you use a provider that is contracted with Guardian. There are two networks you may access. The first is the DentalGuard PPO network and can be accessed directly at www.guardiananytime.com or by calling Guardian at 800.541.7846. The second network is custom only for NCSMIG members. This means the provider will pay in-network ONLY for members of NCSMIG. This list can be accessed by calling our office at 707.445.7126. We strongly suggest that you ask your dentist to submit a “prior authorization request” directly to Guardian for any larger treatment plans you have. This will allow the provider to work out any coverage issues and will let you know your out-of-pocket costs prior to treatment. Plan descriptions may also be accessed through your district’s benefits department. Please note that Dental is a defined-benefit plan, meaning you are provided a maximum payable amount per calendar year. Once you exhaust that amount, you will be responsible for all costs until the next January 1. In addition, we utilize a Dental incentive plan. In order to keep your current incentive level, or move up a level, you will need to have at least one paid visit during the calendar year (January – December). If you miss a year, you will be dropped one level. Incentive levels range from 70-100%.
