Pharmacy

CVS Caremark provides the pharmacy benefits for all NCSMIG plans except for the Pine HSA plan, which is provided through Blue Shield of California. For information on the Pine plan’s pharmacy benefits, please contact Blue Shield of California directly at 855.256.9404 or visit their website at www.blueshieldca.com.

Pharmacy coverage copays vary from plan to plan. CVS Caremark provides overall management of the NCSMIG pharmacy benefit program. While CVS Health provides retail stores throughout the country, you are NOT required to use a CVS branded store. You may use the pharmacy of your choice. However, it is important to verify with your pharmacy, PRIOR to receiving services, that they are contracted with CVS Caremark and can bill your prescription. The easiest way to find out if your pharmacy of choice can bill CVS Caremark is to call and ask. If you need further assistance, you can contact CVS Caremark directly at 844.207.2081. You can also access information on contracted pharmacies, prior claims and your specific plan benefits at www.caremark.com/mycaremark. If your pharmacy of choice needs assistance, they may contact the Pharmacy Help Desk at 800.364.6331 for help.

Prior Authorizations

Some prescriptions for particular medications may require a prior authorization prior to being dispensed. The pharmacy should contact you or your physician if your prescription requires a prior authorization review. We strongly recommend you stay in frequent contact with your physician and make sure they provide enough information for CVS Caremark to render an authorization. Your doctor will be able to provide supporting chart notes and a medical summary allowing CVS Caremark to make a quick decision.

Mail Order

Maintenance medications are provided through the CVS Caremark mail order pharmacy. If you do not want to participate in mail order for your maintenance medications, please make sure you call Customer Service and ask to “opt-out.” We do not require you to only use mail order, it is your choice.

Livongo

Livongo is a fabulous new program offered through CVS Caremark for our members living with Diabetes: Type 1, Type 2 or pre-diabetes. If you qualify for this program, you will receive eligibility information in the mail directly from Livongo. One of the best parts of this coaching program is that you will receive FREE unlimited monthly test strips and lancets mailed directly to your home, personalized coaching and easy access to your data from your phone, computer or tablet. This information can quickly be shared or downloaded to provide to your physician. Monitoring and reacting to your readings, along with a great custom coaching program (including dietary information), can help you manage your diabetes and improve your overall health and wellbeing.
